All-on-4® Treatment Concept
Traditional dentures may help a patient chew, but they’ve never been known for being comfortable. Today, we have solutions to upgrade your dentures and help them look and feel more like your natural teeth. The All-on-4® Treatment Concept combines your denture with the addition of four dental implants to permanently retain your denture in your mouth. This means that your denture stays right where it belongs, in your mouth, without shifting or rocking. Patients receive the feel of their natural teeth and following a period of healing, they can resume their regular diet. Dentures that are retained with just suction or adhesive have long been the topic of frustration and even jokes, that’s because neither of these solutions work well. At Tropic Dental, we specialize in several implant retained full-arch replacement options when replacing a full arch of teeth. One implant retained option is the All On 4 solution. This is a complete fixed bridge that is supported permanently in place by a series of dental implants. The difference between this solution and other implant retained dentures is that your teeth stay firmly in place as a non-removable solution. The All On 4 solution is an immensely beneficial treatment which has revolutionized the way we can replace missing teeth.
How does the All-on-4® Treatment Concept work?
A dental bridge is a system we have used in dentistry to replace missing teeth for decades. It applies false teeth that are retained with anchors. Traditionally, the anchors were healthy teeth, but then we started using dental implants when no healthy teeth were present. A dental implant is post that we surgically insert into the jawbone to restore a solid connection to the jaw. The implant serves as a firm rod or post. The early beginnings of this new concept required between six and eight implants, but the All On 4 can complete the same work with just four implants due to its length and placement. We can now surgically place dental implants to anchor an entire arch of missing teeth with just four implants.
The All On 4 system is a complete bridge that only requires four implants. This is because we use longer implants designed to reach denser bone and the angle they are placed at. The two posterior (back) implants are angled to maximize the support from your jawbone. The two anterior (front) implants are placed at a vertical angle like conventional implant screws for a firm hold.
All On 4 Process
The All On 4 process begins with a consultation, we will want to view the area and collect information through a series of diagnostic tests possibly including x-rays, CBCT scans, and other 3D imaging technology. In addition, we will want to discuss medication, treatments and other information to help us determine candidacy, the patient needs to be able to heal at a reasonable rate which can be impacted by certain conditions and habits.
When surgical treatment has been determined, we will then create a precise treatment map, detailing the ideal placement for the implants. We may also need to remove unhealthy teeth and treat the mouth for infection. Unlike other implant procedures, the All On 4 doesn’t generally require a bone graft prior to placement. This is because we extend the implants to thicker, deeper bone material.
On the day of surgery, after we’ve made sure the patient is comfortable, we will then begin with making four small incisions in the gums to expose the underlying bone. We will create space in the bone with a specialized drill and the treatment map in place. The implants are deeply embedded. Once the implants are in place, we will attach the abutments, which are a support structure and used to link the screws to your restoration. We will then install a prefabricated temporary restoration. The All On 4 treatment allows for same-day restoration placement, though in most cases we begin with a temporary and place the permanent restoration later after healing has occurred.
